504 Procedures

Referrals to 504 follow the RESULTS process as adopted by Wylie ISD (see RESULTS web link)

The teacher documents student needs and communicates that need to the parent, student, and relevant school personnel to formulate a plan of action or obtain recommended strategies to try before making a 504 referral.   Should a disability be suspected a referral to Special Education or 504 may be made.

 

504 Committee Meeting Parent /Student Rights of Due Process of sent home and data is gathered for consideration of eligibility under Section 504.  An appropriate educational intervention plan is developed by the team of knowledgeable persons.


This team may include the following persons:

Parent

Teacher

Administrator

Counselor

Nurse

Student’s progress is reviewed annually and also prior to any change of placement or services, which will last over 10 days.

 

All accommodations outlined on a student’s individual 504 plan MUST be routinely implemented
