Criteria for Transition to Life/Special Programs Center
1. Employability skills: Students must possess the physical & mental ability to be able to learn & improve these skills as a requirement for getting, keeping, & doing well on their job or volunteer site.
a. Dependable in attendance & work tasks
b. Presents a neat & clean appearance
c. Follows directions
d. Maintains a positive attitude toward work
e. Interacts appropriately with adults & peers
f. Respects authority
g. Physical stamina
h. Displays independence in work
i. Requests assistance when needed
j. Remains on-task & works to completion
k. Displays acceptable productivity
l. Controls emotions & accepts criticism
m. Utilizes good decision-making in completing tasks
2. Employment with some assistance must be a realistic goal for the student to succeed in the program
3. Family/Student must be receiving local/state agency assistance or pursuing access to an outside agency for services
(Examples below):
a. Department of Assistive and Rehabilitative Services (DARS)
*May help provide job placement/coaching, counseling, & guidance
b. Texas Department of Human Services (TDHS)
*Can help provide day activities & community living assistance
c. LifePath Systems/Collin Co. MHMR
*Caseworker assistance in accessing resources, such as guardianship
d. Texas Workforce Commission
*Helps with job placement

4. Student must have/obtain their Social Security and state ID card
5. Students must provide their own lunch on a daily basis: *Microwave, refrigerator, & toaster oven are available
6. Family must be willing to aid in transportation if the student’s job requires them to attend an after-school-hours event *For example: training, orientation, drug testing, or weekend work
7. Family/Students must be willing to fulfill job requirements of student’s employer *Appropriate work uniform (no-slip shoes, black or khaki pants, etc.)
