DISABILITY INSURANCE:         

Disability coverage is provided by UNUM Life Insurance Company of America.

Disability coverage provides payment of up to 2/3 of your salary in the event you are ill or injured and cannot work.  The cost depends upon the amount of coverage selected and on how quickly you wish benefits to begin.  A variety of elimination (waiting) periods are available. All new hire employees will be accepted into the plan without having to provide evidence of insurability.  If you are not a new hire, there will be an underwriting process.  Pre-existing conditions will not be covered until after twelve months of continuous coverage. A pre-existing condition is any condition for which advice or treatment was received in the 90 days prior to the effective date of coverage.
