There are two plans available for your choice of coverage.  Both plans are portable upon termination of employment. 

Scheduled Cancer Plan: Benefits are paid in accordance with a schedule of benefits. UTA provides a supplemental plan of insurance that pays benefits directly to you to help offset expenses for the treatment of cancer that are not covered by major medical insurance. An application for coverage must be submitted to the insurance company for approval.   Primary benefits are for the treatment of cancer, but coverage is also available when admitted to the intensive care unit of a hospital for any reason.Review the summary plan description in the Reference Center at www.benefitsolver.com for more details.

First Diagnosis Cancer Plan: UTA also provides a plan that will pay directly to you a lump sum payment upon the first diagnosis of internal cancer.  Amounts of coverage are available from $10,000 to $50,000 for the first diagnosis of internal cancer.  Optional coverage is available for any condition when admitted to an intensive care unit of a hospital. An application for coverage must be submitted to the insurance company for approval.  Refer to the Reference Center at www.benefitsolver.com for more details.

 

2010- 2011 Allstate Insurance 

Allstate Insurance is a new provider of Group cancer insurance as of September 1, 2010. This is a supplemental insurance plan that provides payments directly to you to offset expenses related to the treatment of cancer. The plan pays according to a schedule of benefits and includes a benefit for the initial diagnosis of cancer. Optional coverage is available for any condition when confined to an intensive care unit of a hospital.

For the 2010-2011 benefit year only, every eligible employee and family member who applies for coverage during open enrollment, will be guaranteed coverage; however, a pre-existing condition limitation does apply. A pre-existing condition is a disease or physical condition for which medical advice or treatment was received by the covered person during the 12 month period prior to the effective date of coverage. Allstate does not pay for any loss due to a pre-existing condition during the first 12-months of coverage. If you wish to keep your current cancer plan, you must make payment arrangements with United Teachers Associates (UTA) directly by calling 1-800-880-8824. Your UTA Group number is AC688. Current plans will not be payroll deducted after September 1, 2010.
