There are two plans available for your choice of coverage. Both plans are portable upon termination of employment.
Scheduled Cancer Plan: Benefits are paid in accordance with a schedule of benefits. UTA provides a supplemental plan of insurance that pays benefits directly to you to help offset expenses for the treatment of cancer that are not covered by major medical insurance. An application for coverage must be submitted to the insurance company for approval. Primary benefits are for the treatment of cancer, but coverage is also available when admitted to the intensive care unit of a hospital for any reason.Review the summary plan description in the Reference Center at www.benefitsolver.com for more details.
First Diagnosis Cancer Plan: UTA also provides a plan that will pay directly to you a lump sum payment upon the first diagnosis of internal cancer. Amounts of coverage are available from $10,000 to $50,000 for the first diagnosis of internal cancer. Optional coverage is available for any condition when admitted to an intensive care unit of a hospital. An application for coverage must be submitted to the insurance company for approval.
