2008 Kindergarten Round Up information

TIME:    4:00p.m. - 7:00p.m.
LOCATION: Your child’s elementary campus

Wylie Independent School District offers a full-day instructional program for students who are five-years old on or before September 1, 2008.

ELIGIBILTY: The child must be five-years old on or before September 1, 2008.

TRANSPORTATION: Kindergarten students may be eligible for transportation. Eligibility for transportation can be determined using the following link: http://www.wylieisd.net/buses/schedule.htm

SCHOOLS HOURS: 7:45 a.m. – 2:45 p.m.

REQUIRED REGISTRATION DOCUMENTS:

A copy of the child’s official birth certificate
A complete, up-to-date, immunization record signed by physician and/or clinic
Proof of residency
Child's Social Security Card
Parent’s driver’s license

IMMUNIZATIONS FOR ENROLLMENT 2008 - 2009:

Kindergarten Requirements

5 doses of DTP/DTaP Vaccine (4 if 4th dose is after 4th birthday)

4 doses of Polio Vaccine (3 if 3rd dose is after 4th birthday)

2 doses of MMR Vaccine, 1st dose on or after 1st birthday

3 doses of Hepatitis B Vaccine

1 dose of Varicella Vaccine on or after 1st birthday OR Serological proof of immunity or documentation of previous chickenpox illness

Hib ( A series and booster completed by 15 mo. or 1 dose completed after age 15 mo.) for children 15 mo. through 4 years old
