Vaccinate Before You Graduate campaign

The Care Van will be at Wylie High School on April 14, 2009.  They will be providing FREE immunizations to WHS high school juniors and seniors.  They will have all of the vaccines required by the Texas Department of State Health Services, as well as the meningococcal vaccine, Menactra.  Many universities require or recommend meningococcal vaccine prior to enrollment.   

There is a limited supply of Menactra, therefore the permission forms for the immunization must be signed and returned to the high school nurse no later than April 9, 2009.

No immunization will be given without parent permission forms signed. The application and forms required are for the Care Van records; we do not keep that information at the school.
